Submitting Purchase Orders

Bungalow Software accepts Purchase Orders from Medical and Educational Institutions (hospitals, schools, etc.), subject to a credit check.

Instructions

1. Refer to our catalog for prices and item numbers.
2. PO orders must be shipped Priority  so the shipment can be tracked.
3. Email your Institutional Purchase Order to email, or mail it to:
   • Bungalow Software  PO BOX 11638  Blacksburg, 24062-1638
4. Call phone and leave your name, phone number and the PO #.  If the PO doesn't arrive for some reason, we'll contact you.
5. Our Federal Tax ID # is : Fed Tax ID Number.
   Our DUNS number is: 02-396-9038
   You can print a copy of our W9 form and VA Vendor Form.

We reserve the right to refuse a Purchase Order from any institution with an unsatisfactory credit history.
